If your private information was in the possession of Affordacare and/or Wichita Medical Management and potentially impacted by the cybersecurity incident involving Affordacare computer systems, you may be eligible for relief from a class action settlement.
A settlement has been proposed in a class action lawsuit against C, L and W PLLC d/b/a Affordacare, (“Affordacare”) relating to a cybersecurity incident involving Affordacare computer system in February 2020 (the “Data Incident”). The computer systems affected by the Data Incident potentially contained certain personal and protected health information (such as Social Security numbers, health insurance information, and other protected health information) relating to Affordacare patients.
If you are an individual whose Private Information was received, gathered, shared, obtained, or otherwise found itself in the possession of Affordacare and/or Wichita Medical Management (“WMM”) and was potentially impacted by the cybersecurity incident involving the computer systems of Affordacare discovered on or about February 3, 2020, you are a part of the class, and may be eligible for a payment.
The Settlement provides for identity theft protection services to Settlement Class Members who claim them.
